I voluntarily assume full responsibility for any risks, injuries, allergic reactions, or damages that may occur during or as a result of my participation. Release of Liability I hereby release, waive, discharge, and hold harmless Blaze and Create, From My Heart Beauty & Wellness LLC, its owners, staff, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, illness, accident, or damages that may occur during or after the Event.
